Just like with our Skilled Home Health services, you’ll start with a Case Manager who works with you to create a personalized Plan of Care based on your unique needs and lifestyle.
What Is Community Health Care?
Unlike skilled nursing or therapy, Community Health Care focuses on non-medical (custodial) support. These services are not covered by Medicare or private insurance, but they may be eligible for county waiver programs or can be arranged through private pay.
Compassionate Professionals by Your Side
Our care team includes licensed professionals and compassionate caregivers who work together to deliver personalized, coordinated support—focused on your health, comfort, and independence at home.
Nurses
- Visit your home to assess your care needs.
- Review your medications for safety and accuracy.
- Set up weekly or monthly med planners.
- Provide medication management if needed.
Home Health Aides (HHAs)
- Medication reminders or administration. (as allowed)
- Assistance with bathing, dressing, and grooming.
- Light housekeeping and meal preparation.
- Support with walking and mobility.
- Provide supervision and companionship.
